Request Form for Private Mass(es) to be celebrated for the
Healing of the Family Tree (or any other intention or group of intentions)
Some petitioners like to submit a genogram, although there is no need to specify the name of the family or individuals for whom the Masses are requested, since God knows those persons and their needs (see Matthew, 6:32).
However, for communication purposes, the petitioner’s name, address, etc. should be filled out as completely as possible. Stipends are used to support the poor of our Claretian missions. This completed form (printed out or copied) may be submitted with the stipend offering.
